Certainly, however patients may not always have the option to choose their healthcare provider due to provider availability. During your appointment, you will receive care from either a nurse practitioner or a doctor who is available at that time. We recommend contacting our office in advance to ascertain appointment availability and the assigned healthcare provider.

Yes, we do provide telehealth or virtual consultation services.

To reschedule or cancel an appointment, kindly contact our office via phone or text message at 702-877-9511 with a minimum of 24 hours; notice prior to your scheduled appointment. Your prompt notification allows us to accommodate other patients efficiently.

Patients are requested to bring their identification, insurance cards, and a comprehensive list of their current medications. If applicable, we also encourage patients to bring any relevant medical records obtained from hospitals or specialists. Your cooperation in providing
these documents will greatly assist us in delivering the most effective and informed care during your visit.

To change pharmacy, you can text us the name and address of your preferred pharmacy, and we will promptly update our records accordingly.

To obtain your prescription refill, kindly send a text message to 702- 877-9511 with the following details in the specified format: Your full name, date of birth, prescribed medication, dosage, frequency, and your preferred pharmacy. For instance, your message should
resemble the following example:

  • Full Name: John Doe
  • Date of Birth: 01/01/2001
  • Medication: Metformin
  • Dosage: 500mg
  • Frequency: Twice a day

    This procedure will facilitate the prompt processing of your medication refill request.

Controlled medications undergo stringent regulation and oversight at the state level, necessitating MONTHLY EVALUATION by a healthcare provider for prescription renewal. These prescriptions are limited to a 30-day worth of supply per visit. The most common controlled medications subject to these regulations include:

Please be aware that the prescribing and monitoring of these medications strictly adhere to state regulations to ensure the safety and well-being of patients.

  • Certainly, our clinic is equipped with an in-house laboratory, conveniently situated in Suite #1. The laboratory operates during the hours of Monday to Friday, from 7:45 am to 3:00 pm. There are scheduled breaks from 10:00 am to 10:30 am and a lunch break from 12:00 pm to 1:00 pm. Please note that Our laboratory exclusively processes orders originating from within
    our own facility.
  • Our laboratory services are available on a walk-in basis, and appointments are not necessary. To utilize our laboratory services, kindly present a valid ID and your insurance card at the front window. If you do not possess a lab order, our front desk staff will be pleased to assist you by printing one for your convenience.
  • Please be aware that certain blood work tests necessitate a minimum fasting period of 8 hours, during which you should abstain from consuming food. However, you may still drink water and take prescribed medications during this fasting period. We recommend accomplishing your blood draw at least
    5 days in advance of your upcoming appointment with your healthcare provider to ensure timely and accurate results.

Our clinic offers services in English and Tagalog, and we also provide interpreter services for Spanish-speaking patients.

The referral processing timeline typically spans 3 to 5 business days. Kindly note that the approval of your referral will be issued by your
insurance provider.

The completion of the Family and Medical Leave Act (FMLA) process requires a standard timeframe of five days, and there is a nominal fee of $35 associated with this service.

The retrieval and processing of medical records typically require a time frame of 10 to 14 business days.

The processing of DME (Durable Medical Equipment) or prior authorizations typically requires approximately 7 to 14 days for completion through your insurance provider. We kindly request that you proactively contact them if you have not received a response
within the initial two-week period.

The suggested frequency of medical appointments may differ based on factors such as your age, overall well-being, and individual risk factors or medical conditions. For individuals in good health, it is typically advisable to schedule appointments at least once or twice annually.

We recommend seeking immediate medical attention at the nearest urgent care facility. In the event of a life-threatening emergency, please dial 911 or proceed to the nearest emergency care center.

  • For individuals seeking to establish care with our clinic as new patients, the fee is $175 for self-pay patients. For established
    patients returning for follow-up appointments, the fee is $125. Additionally, we offer the convenience of telehealth visits for $80.
  • We accept various payment methods to accommodate your preferences, including cash and major credit cards such as Visa, Mastercard, and American Express. Your ease of payment is important to us, and we are here to assist you with any financial inquiries you may have.
